• The Expert Ally LLC is engaged to provide an IEP review for a minor. I understand that by obtaining this service I certify that I am a legal guardian of said minor and am authorized to share their confidential records.

• I understand that the service provided involves a review of the child's records and a 50-min oral feedback session.

• I agree to pay the non-refundable fee of $365. 

​

• I understand that any and all of the services provided by The Expert Ally LLC do not constitute a clinical/psychotherapy practice nor create a clinician-patient relationship between the parties herein and/or The Expert Ally LLC.

• I understand that The Expert Ally LLC assures that all information, conversations, and/or concerns will be kept strictly confidential and only released or shared in compliance with a written authorization or as state or federal law may require.

 

• I understand that if I wish to cancel or change a scheduled feedback session, I must notify The Expert Ally LLC at least 48 hours in advance. Failure to notify will forfeit that appointment and the fee for it. I understand that the fee for a 50-min feedback session is $165.

• I understand that I am responsible to provide a complete set of records. No records will be reviewed after the feedback session.
